Required Forms

“My son and I had our cholesterol levels screened during the health fair in August by one of your nurses. My son’s levels were very high, 400, and mine were significantly high too, 275. The nurse advised us to go to our doctor and have a cholesterol test done while fasting, which we did. My son is now on medication and we are exercising and on a very healthy diet.
